ENROLLMENT INFORMATION

Registration/Enrollment:

Any new student wishing to enroll at Lake Forest should bring the following information with them at the time of enrollment:

1.  Proof of address in the form of an electric bill, phone bill, water bill, gas bill, rent or lease receipt, or house contract.

2.  A copy of student's immunization record from the Health Dept.  (Students coming from out of state should obtain a Tennessee immunization form from the Health Dept. or from a physician.)

3.  A copy of student's birth certificate and social security card.

4.  Current grades from the previous school.

5.  A copy of custody papers (if custody is an issue).

6.  A copy of the current IEP (if your student received any special services at their previous school).

If you are enrolling after the first day of school, you must withdraw your student from his/her current school before applying for enrollment.  Bring a copy of the withdrawal with you.
